The referring physician insisted for percutaneous true cut biopsy .Most IR consultants in the department refused to do the procedure . Finally the consultant cardiologist contacted me to do the procedure .
Under CT guidance when I inserted coaxial needle into the upper part of the mediastinal mass . After needle insertion I noticed pulsatile movement of the needle with heart beats . When I removed the inner stellate of the co axial needle rapid gush of pulsatile blood similar to usual arterial puncture was seen indicating that there was a connection between the heart blood and the cavity of the mass . The procedure was stopped and no biopsy was taken to avoid complications such as bleeding .
I will discuss similar cases in literatures and what will the best protocol in such cases . |
